Abstract: การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาระบบบริการสุขภาพ คุณภาพชีวิต และปัจจัยส่วนบุคคล ที่มีความสัมพันธ์กับคุณภาพชีวิตของผู้ต้องขังในทัณฑสถานโรงพยาบาลราชทัณฑ์ ดาเนินการวิจัย 2 ขั้นตอน คือ 1. ศึกษาระบบบริการสุขภาพในทัณฑสถานโรงพยาบาลราชทัณฑ์ กลุ่มตัวอย่างเป็นบุคลากรตำแหน่งผู้บริหาร หรือผู้ที่มีส่วนเกี่ยวข้องในระบบบริการสุขภาพจำนวน 5 คน 2. ประเมินคุณภาพชีวิตของผู้ต้องขังในทัณฑสถานโรงพยาบาลราชทัณฑ์ กลุ่มตัวอย่างเป็นผู้ต้องขังป่วยในที่เข้ารับการรักษาในทัณฑสถานโรงพยาบาลราชทัณฑ์จนเสร็จสิ้นกระบวนการรักษา และอยู่ระหว่างการส่งตัวกลับเรือนจำ/ทัณฑสถาน จำนวน 193 คน เก็บรวบรวมข้อมูลระหว่างวันที่ 2 20 พฤศจิกายน 2563 โดยใช้เครื่องมือแบบสัมภาษณ์และแบบสอบถาม วิเคราะห์ข้อมูลคุณภาพด้วยการวิเคราะห์เนื้อหา และข้อมูลเชิงปริมาณด้วยสถิติพรรณา ส่วนเบี่ยงเบนมาตรฐาน และสถิติไคสแควร์ ผลการศึกษาพบว่า 1. การบริหารงานด้านโครงสร้างทางกายภาพเอื้อต่อการควบคุม และการให้บริการสุขภาพ มีการบริหารงานด้านการดูแลสุขภาพที่เหมาะสมและสอดคล้องตามความจาเป็นควบคู่กับหลักทัณฑปฏิบัติ และมีการบริหารงานระบบบริการสุขภาพที่ครอบคลุมทั้งการส่งเสริมสุขภาพ การป้องกันโรค การรักษาพยาบาล และการฟื้นฟูสุขภาพ 2. กลุ่มตัวอย่างผู้ต้องขังในทัณฑสถานโรงพยาบาลราชทัณฑ์มีระดับความคิดเห็นเกี่ยวกับคุณภาพชีวิตโดยรวมอยู่ในระดับปานกลาง (mean = 68.51, S.D. = 10.07) ปัจจัยส่วนบุคคล ได้แก่ เพศ และโรคประจาตัวที่ได้รับการวินิจฉัยจากแพทย์ มีความสัมพันธ์กับคุณภาพชีวิต ของผู้ต้องขังในทัณฑสถานโรงพยาบาลราชทัณฑ์อย่างมีนัยสำคัญทางสถิติ (p-value < .01) ข้อเสนอแนะจากการวิจัย ผู้บริหารควรส่งเสริมและสนับสนุนการพัฒนาบุคลากรในสาขา ที่ขาดแคลน เพื่อให้บุคลกรมีความรู้ ความสามารถในการปฏิบัติงาน ลดการส่งตัวผู้ต้องขังป่วยออกไปรับการรักษาภายนอก นอกจากนี้ควรมีนโยบายในการส่งเสริมสุขภาพจิตที่ดีให้แก่ผู้ต้องขัง รวมถึงการจัดการข้อมูลข่าวสาร ที่จำเป็นให้ แก่ผู้ต้องขังอย่างเหมาะสม

Abstract: The purposes of this research were to study health service system, and evaluating the prisoners' quality of life in a medical correctional institution. This study was conducted in two steps: first step was collecting data from 5 administrators or personnel who had experiences with health service system of a medical correctional institution; second step was collecting data from 193 prisoners who were admitted in a medical correctional institution, until treatment process was completed and in process to transfer back to prison. Data collection was conducted from 2-20 November 2020. A questionnaire was used as the data collection instrument from prisoners. Descriptive statistical analysis included frequency, percentage, mean, standard deviation and chi-square. Data from personnel group was collected by interview, with content analysis. The study found that 1. The management of physical structure was conducive to control and provide health services; the management of health services was appropriate, and in accordance with necessity along with the principles of practice, and the management of health services system, covering health promotion, disease prevention, treatment and rehabilitation; 2. The overall quality of life among the sample group was at a medium level (mean = 68.51, S.D. = 10.07). The personal factors which showed statistical significance (p-value < .01) were gender and underlying personal illness. Based on the research results, recommendations are as follow. First, administrators should be supportive and encouraging knowledge development of personnel, in order to decrease referral cases. Second, there should be a mental health promotion policy, and appropriate information management for prisoners.
