Effectiveness of mobile phone application-based social support for chinese stroke caregivers: A Randomized controlled trial

Abstract: Stroke is a major public health concern worldwide. Stroke caregivers’ unpreparedness to perform caregiving for their stroke survivors contributes to negative mental health consequences like caregiver burden and depression. Therefore, enhancing care preparedness is essential for stroke caregivers to reduce these consequences. This randomized controlled trial aimed to evaluate the effectiveness of the Mobile Phone Application-Based Social Support program on caregiver burden and depression for stroke caregivers. Participants were 80 stroke caregivers of stroke survivors who received treatment at Zhenjiang First People’s Hospital, Jiangsu Province, China and met the inclusion criteria. All participants were randomly assigned equally into an experimental group or a control group (40 persons each). The experimental group received the Mobile Phone Application-Based Social Support program developed by the researcher based on the trajectory conceptual framework combined with social support, whereas the control group received usual care practices. Data were collected using the Chinese version of the Zarit Burden Interview (ZBI-C) immediately after the program’s end and the Chinese version of the patient health questionnaire-9 (PHQ-9-C) at baseline and at one month post-program. Data were analyzed using descriptive statistics, Chi-square test, independent sample t-test, paired sample t-test, and Mann-Whitney U test. The study results were as follows: 1. Participants receiving the Mobile Phone Application-Based Social Support program had significantly lower caregiver burden immediately after the program than those receiving usual care practices (p < .05). 2. Participants receiving the Mobile Phone Application-Based Social Support program had significantly lower depression at one month post-program than before and lower depression than those receiving usual care practices at one month post-program (p < .05). The findings of this study provide new knowledge about a trajectory-based support program for stroke caregivers that nurses can use to decrease caregiver burden and depression by improving care preparedness.

Abstract: โรคหลอดเลือดสมองเป็นปัญหาสุขภาพที่สำคัญทั่วโลก ความไม่พร้อมของผู้ดูแลในการดูแล ผู้ที่รอดชีวิตจากโรคหลอดเลือดสมองส่งผลกระทบทางลบต่อภาวะสุขภาพจิตของผู้ดูแลเช่น ภาระในการดูแลและภาวะซึมเศร้า ดังนั้นการส่งเสริมความพร้อมในการดูแลจึงเป็นสิ่งจำเป็นสำหรับผู้ดูแลผู้ป่วยโรคหลอดเลือดสมองในการลดภาระการดูแลและภาวะซึมเศร้า การทดลองแบบสุ่มครั้งนี้มีวัตถุประสงค์เพื่อประเมินประสิทธิผลของการสนับสนุนทางสังคมบนแอพพลิเคชันมือถือต่อภาระในการดูแลและภาวะซึมเศร้าสำหรับผู้ดูแลผู้ป่วยโรคหลอดเลือดสมอง กลุ่มตัวอย่างเป็นผู้ดูแลผู้ป่วยโรคหลอดเลือดสมองที่รับการรักษาที่โรงพยาบาลประชาชนที่ 1 เซนเจียง จังหวัดเจียงซู สาธารณรัฐประชาชนจีน จำนวน 80 รายที่ผ่านเกณฑ์การคัดเข้า โดยกลุ่มตัวอย่างทั้งหมดถูกสุ่มเข้ากลุ่มทดลองและกลุ่มควบคุมจำนวนกลุ่มละ 40 ราย กลุ่มทดลองได้รับการสนับสนุนทางสังคมบนแอปพลิเคชันมือถือที่พัฒนาโดยผู้วิจัยจากแนวคิดวิถีทางการเจ็บป่วยร่วมกับการสนับสนุนทางสังคม ในขณะที่กลุ่มควบคุมได้รับการดูแลตามปกติ การเก็บรวบรวมข้อมูลใช้แบบสอบถามภาระในการดูแลของซาริท ฉบับภาษาจีน (The Chinese version of Zarit Burden Interview) ภายหลังสิ้นสุดโปรแกรม แบบสอบถามภาวะสุขภาพ 9 ฉบับภาษาจีน (The Chinese version of patient health questionnaire-9) ก่อนและหนึ่งเดือนหลังได้รับโปรแกรม วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา การทดสอบไควสแควร์ สถิติทดสอบที ชนิดสองกลุ่มเป็นอิสระและไม่เป็นอิสระ และสถิติแมนวิทนีย์ยู ผลการศึกษาพบว่า 1. กลุ่มตัวอย่างที่ได้รับโปรแกรมการสนับสนุนทางสังคมบนแอปพลิเคชันมือถือมีภาระการดูแลที่ลดลงภายหลังสิ้นสุดโปรแกรมทันทีเมื่อเทียบกับกลุ่มที่ได้รับการดูแลตามปกติ อย่างมีนัยสำคัญทางสถิติ (p < .05) 2. กลุ่มตัวอย่างที่ได้รับโปรแกรมการสนับสนุนทางสังคมบนแอปพลิเคชันมือถือมีภาวะซึมเศร้าที่ลดลงที่หนึ่งเดือนหลังสิ้นสุดโปรแกรมเมื่อเทียบกับก่อนได้รับโปรแกรม และเมื่อเทียบกับกลุ่มที่ได้รับการดูแลตามปกติที่หนึ่งเดือนหลังสิ้นสุดโปรแกรม อย่างมีนัยสำคัญทางสถิติ (p < .05) ผลการศึกษาครั้งนี้ให้ความรู้ใหม่ของโปรแกรมการสนับสนุนตามแนวคิดวิถีการเจ็บป่วยสำหรับผู้ดูแลผู้ป่วยโรคหลอดเลือดสมอง ซึ่งพยาบาลสามารถนำไปใช้เพื่อการลดภาระการดูแลและภาวะซึมเศร้าโดยผ่านการส่งเสริมความพร้อมในการดูแล
