A Retrospective study of suspected canine pyometra at the small animal teaching hospital, Chiang Mai University, Thailand during 2016 - 2018

Abstract: The disease which causes of economical loss and might be death in intact female dog is pyometra. The objective of this study was evaluated of the proportion, incidence of suspected canine pyometra, mortality rate, the factor relates suspected pyometra dog’s death, and cost of treatment during 2016 to 2018 at the small animal teaching hospital, Chiang Mai university. During the study period, there were 35,138 female dogs visiting to the hospital and 375 dogs were suspected with pyometra (1.07 %). 345 dogs were undergoing surgery at the hospital. The age of dogs was ranged by 11 months old to 16 years old (median was seven years). The most presented breed of pyometra dogs were mongrel breed (134/345, 37.33%), Poodle (51/345,14.67%) and Shih tzu (43/345,12.27%), respectively. There were three dogs died before surgery. Thirty-four dogs from 345 dogs (9.86%) died after surgical treatment: nine dogs died within 12 hours after surgery, seven dogs died between 12 hours and one week, eight dogs died more than one week and 10 dogs no data about death time. The factor relate suspected pyometra’s dog death is uterine rupture and also the most predisposing cause of uterine problem is injected medroxyprogesterone acetate as contraceptive drug. The cost of treatment about 4,856.97 ± 1,938.97, 8,236.69 ± 4,348.18, and 8,565.35 ± 5,038.78 baht, in year 2016 to 2018, respectively From this study, the veterinary and the dog’s owner should concern about the uterine diseases in intact female dog. To prevention the disease by schedule surgical ovariohysterectomy (OVH) in young dog is important. To decrease the mortality, peri-operative intensive care, close monitoring the clinical signs and also performing blood examinations are recommended. Moreover, avoid using contraceptive drug should educate to the owner.

Abstract: โรคในสุนัขที่เกิดขึ้นแล้วส่งผลเสียหายทางเศรษฐกิจ และอาจเสียชีวิตได้ในสุนัขเพศเมียที่ยังไม่ทำหมันคือ โรคมดลูกอักเสบ จุดประสงค์ของการศึกษานี้คือ ศึกษาสัดส่วนของการเกิดโรค อุบัติการณ์ของการเกิดโรคมดลูกอักเสบ อัตราการตาย และปัจจัยที่มีความสัมพันธ์กับการเสียชีวิตในสุนัขที่สงสัยโรดลูกอักเสบ และค่าใช้จ่ายในการรักษาโรคนี้ในสุนัข ระหว่าง พ.ศ. 2559 ถึง พ.ศ. 2561 ที่โรงพยาบาลสัตว์เล็กเพื่อการเรียนการสอน มหาวิทยาลัยเชียงใหม่ ตลอดการศึกษาพบสุนัขตัวเมีย 35,138 ตัวที่เข้ามารับการรักษาที่โรงพยาบาล และพบสุนัขที่สงสัยมดลูกอักเสบ 375 ตัว (1.07 %). มีสุนัขจำนวน 345 ตัวที่รับการรักษาด้วยการผ่าตัด พบว่าสุนัขที่สงสัยมมดลูกอักเสบมีอายุระหว่าง 11 เดือน ถึง 16 ปี (ค่ามัธยฐาน 7 ปี) และพันธุ์ที่พบมากที่สุด ได้แก่ พันธุ์ทาง (134/345, 37.33%), พุดเดิ้ล (51/345, 14.67%) และชิสุห์ (43/345, 12.27%), ตามลำดับ พบว่าสุนัขสังสัยมดลูกอักเสบเสียชีวิตก่อนการรักษาด้วยการผ่าตัด 9 ตัว และมีสุนัข 34 ตัว (9.86%) จาก 345 ตัวเสียชีวิตหลังจากการผ่าตัด โดยเสียชีวิตภายใน 12 ชั่วโมงหลังผ่าตัด, 7 ตัว เสียชีวิตระหว่าง 12 ชั่วโมงแต่ไม่เกิด 1 สัปดาห์, 8 ตัวเสียชีวิตมากกว่า 1 สัปดาห์หลังการผ่าตัด และ 10 ตัว เสียชิวิตโดยไม่ทราบช่วงเวลาที่แน่ชัด ปัจจัยที่มีความสัมพันธ์กับการเสียชีวิตของสุนัขสงสัยมดลูกอัเสบคือ มดลูกแตก และสาเหตุโน้มนำปัญหาเกี่ยวกับมดลูกที่พบมากที่สุด คือการฉีดเมดรอกซีโปรเจสเตอโรน อะซิเตรด ที่ทำหน้าที่เหมือนยาคุมกำเนิด และ ค่าใช้จ่ายในการรักษามดลูกอักเสบในสุนัข ประมาณ 4,856.97 ± 1,938.97, 8,236.69 ± 4,348.18, และ 8,565.35 ± 5,038.78 บาท, ระหว่าง พ.ศ. 2016 ถึง 2018, ตามลำดับ จากการศึกษานี้ สัตวแพทย์ และเจ้าของสัตว์ควรคำนึงถึง ปัญหาที่เกี่ยวข้องกับมดลูกในสนัขเพศเมียที่ยังไม่ได้ทำหมัน เพื่อป้องกันโรค ควร นำสุนัขเพศเมียไปทำหมัน เพื่อลดอัตราการเสียชีวิต ในระหว่างการรักษา ตลอดจนการดูแลภายหลังการผ่าตัดอย่างใกล้ชิด ติดตามอาการ และผลเลือด นอกจากนี้ควรให้ความรู้แก่เจ้าของสัตว์ เพื่อให้ตระหนักถึงการนำสุนัขไปทำหมันแทนการใช้ยาคุมกำเนิด
