Abstract:
The purpose of this study was to define the meaning of Quality of Life in Thai patients with post MI., develop the Quality of Life Instrument in Thai patients with Post Myocardial Infarction (MI), test the psychometric properties of the instrument, and find the norms of QOL in Thai patients with post MI. In developing the new instrument, qualitative study was used to define the meaning and the construct of QOL by seventeen patients with post MI. An item pool was developed based on the meaning and the initial items were sixty eight. The face validity was done by thirteen experts from various disciplines. After the revision according to the comments from the experts, sixty two items were included in this instrument. Reliability tests were done by thirty participants and the result was .928. Data collection was conducted from five hundred and twenty six participants with post MI from various parts of Thailand, responding to the QOL instrument in Thai patients with post MI and other two QOL instruments: SF36 Health Survey, and MacNew Heart Disease Health Related Quality of Life Questionnaire. Data was analyzed by SPSS version 14. Construct validity of the QOL instrument in Thai patients with post MI was determined by factor analysis. Exploratory factor analysis (EFA) indicated that the instrument consisted of nine correlated factors which were analyzed from five hundred and twenty six participants. The result, the Quality of Life Instrument in Thai patients with Post MI has 9 factors, 45 items Likert's scale instrument measuring perception of quality of life in nine dimensions: 1) Symptom and Complication, 2) Psychological Comfort, 3) Family Ties 4) Adapted ADL 5) Economic Stability, 6) Spiritual Health, 7) Social Engagement, 8) Basic Physical Capacity, and 9) Feeling Empowered. The reliability of this instrument is .93 Cronbach's alpha coefficient. The criterion related validity between Thai QOL instrument and SP36 Health survey is .618 and between Thai QOL instrument and MacNew HRQOL Questionnaire is .645. Norms of QOL in Thai patients with post MI has five levels: very good, good, fair, poor, and very poor. The result indicated that the QOL instrument in Thai patients with post MI had nine dimensions which consisted of 45 items. It had the quality of the test in face validity and reliability, and had the T-score that was the norm of Thai patients with post MI. This instrument is a valuable instrument for assessing QOL in post MI patients in clinical studies or futuer studies into the measurement of QOL.

การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อหาความหมายของคำว่าคุณภาพชีวิตของผู้ป่วยไทยโรคกล้ามเนื้อหัวใจขาดเลือดพัฒนาและทดสอบเครื่องมือวัดคุณภาพชีวิตของผู้ป่วยไทยโรคกล้ามเนื้อหัวใจขาดเลือด และหาเกณฑ์ปกติของคุณภาพชีวิตของผู้ป่วยไทยโรคกล้ามเนื้อหัวใจขาดเลือด การพัฒนาเครื่องมือนี้เริ่มด้วยการหาความหมายและโครงสร้างของคุณภาพชีวิต โดยใช้วิธีการศึกษาเชิงคุณภาพในผู้ป่วยไทยโรคกล้ามเนื้อหัวใจขาดเลือด 17 คน และนำข้อมูลที่ได้มาสร้างข้อคำถามได้ทั้งหมด 68 ข้อ ตรวจสอบความตรงปรากฏ โดยผู้เชี่ยวชาญ 13 ท่าน จากหลากหลายสาขาวิชา ภายหลังจากตรวจสอบทบทวนข้อเสนอแนะของผู้เชี่ยวชาญ ข้อคำถามของคุณภาพชีวิตปรับเหลือ 62 ข้อ ทดสอบเครื่องมือกับผู้ป่วยไทยโรคกล้ามเนื้อหัวใจขาดเลือด 30 คน ได้ค่าความเที่ยงของเครื่องมือเท่ากับ .928 การรวบรวมข้อมูลจากผู้ร่วมโครงการวิจัยซึ่งเป็นผู้ป่วยโรคกล้ามเนื้อหัวใจขาดเลือด จำนวน 526 คน จากภูมิภาคต่างๆ ทั่วประเทศ โดยตอบแบบสอบถามในเรื่องคุณภาพชีวิต 3 ชุด คือ แบบสอบถามสร้างใหม่ แบบสอบถาม เอสเอฟ 36 และแบบสอบถามคุณภาพชีวิตแมคนิวส์ การวิเคราะห์ข้อมูลใช้โปรแกรมทางสถิติ SPSS 14 และตรวจสอบความตรงเชิงโครงสร้างด้วยวิธี การวิเคราะห์องค์ประกอบเชิงสำรวจ การรวบรวมข้อมูล จากผู้ร่วมโครงการวิจัยซึ่งเป็นผู้ป่วยโรคกล้ามเนื้อหัวใจขาดเลือด จำนวน 526 คน จากภูมิภาคต่างๆ ทั่วประเทศ โดยตอบแบบสอบถามในเรื่องคุณภาพชีวิต 3 ชุด คือ แบบสอบถามสร้างใหม่ แบบสอบถาม เอส เอฟ 36 และ แบบสอบถามคุณภาพชีวิตแมคนิวส์ การวิเคราะห์ข้อมูลใช้โปรแกรมทางสถิติ SPSS 14 และตรวจสอบความตรงเชิงโครงสร้างด้วยวิธีการวิเคราะห์องค์ประกอบเชิงสำรวจ ผลการวิจัย เครื่องวัดคุณภาพชีวิตของผู้ป่วยไทยโรคกล้ามเนื้อหัวใจขาดเลือด มี 45 ข้อคำถาม เป็นแบบประมาณค่า 5 ระดับ มี 9 องค์ประกอบ คือ 1) อาการของโรคและภาวะแทรกซ้อน 2) จิตใจที่สบาย 3) ความผูกพันในครอบครัว 4) การปรับกิจกรรมประจำวัน 5) ความมั่นคงทางเศรษฐกิจ 6) สุขภาพทางจิตวิญญาณ 7) การมีส่วนร่วมในสังคม 8) ความสามารถพื้นฐานของทางร่างกาย และ 9) กำลังใจค่าความเที่ยงของเครื่องมือเท่ากับ .93 เครื่องมือวัดคุณภาพชีวิตที่สร้างขึ้นได้รับการตรวจสอบความตรงตามเกณฑ์สัมพันธ์ โดยเปรียบเทียบกับเครื่องมือวัดคุณภาพชีวิตแบบทั่วไป SF36 ได้ค่าความสัมพันธ์ .618 มีนัยสำคัญทางสถิติที่ระดับ .01 และเมื่อเปรียบเทียบกับเครื่องมือวัดคุณภาพชีวิตแบบเฉพาะได้ค่าความสัมพันธ์ .645 มีนัยสำคัญทางสถิติที่ระดับ .01 เกณฑ์ปกติของคุณภาพชีวิตของผู้ป่วยไทยโรคกล้ามเนื้อหัวใจขาดเลือดมี 5 ระดับ คือ ดีมาก ดี ปานกลาง ต่ำ และต่ำมาก ผลการวิจัยนี้สรุปได้ว่า เครื่องมือวัดคุณภาพชีวิตของผู้ป่วยไทยโรคกล้ามเนื้อหัวใจขาดเลือด มีองค์ประกอบสำคัญ 9 องค์ประกอบ 45 ข้อคำถาม เครื่องมือมีความตรงปรากฏ ความตรงเชิงโครงสร้าง ความตรงตามเกณฑ์ มีความเที่ยงและมีเกณฑ์ปกติของคุณภาพชีวิต 5 ระดับ เครื่องมือนี้มีคุณสมบัติเพียงพอสามารถใช้วัดคุณภาพชีวิต ได้อย่างมีประสิทธิภาพ ในการศึกษาวิจัยเกี่ยวกับการวัดคุณภาพชีวิตผู้ป่วยไทยโรคกล้ามเนื้อหัวใจขาดเลือดในอนาคต
