Abstract:
The purpose of this independent study was to study efficiency of
this present waiting system of diabetes clinic, Thasala hospital and present
a development of queuing system of this clinic. With for searching for average waiting time of each step, the average total time and the average patient of waiting by the simulation of the system for reducing the average waiting time.
Instruments of the study are patient interviews and observation.
Data collection was conducted during 21 September 2011 to 21 October 2011.
Total patients for this period are 1,545 persons. Researcher collected data regarding period, frequency of service taking of patient and service system of diabetes clinic and calculated arrival rate and service rate of diabetes clinic. Then simulation
by using Arena program was to find the efficiency of queuing system management and developed the queuing system of diabetes clinic.
The findings show that: arrival rate is Possion distribution and time usage of all step of diabetes clinic is exponential distribution with average service time
per patient for card submitting 0.34 minutes, blood test 1.12 minutes, patient history investigation 1.73 minutes, diagnosis 2.02 minutes and appointment card receiving 2.49 minutes.
From simulation by using Arena program was to find efficiency waiting system management of Thasala hospital, it was found that the average waiting time 250.66 minutes and the average total time 258.27 minutes. The longest waiting time in queue was detected at diagnosis step 141.47 minutes. The next is history investigation step 54.20 minutes, card submitting 30.10 minutes, appointment card receiving 15.49 minutes and the last is blood test 9.28 minutes and the average patient of waiting in this system in diabetes clinic is 68.69 persons with the total patient of waiting for diagnosis is highest with 37.64 persons. The next is history investigation is 14.35 persons, and then is card submitting step is 8 persons and
card appointment receiving is 4.15 persons and the last blood test is 2.52 persons.
From the simulation of present system, researcher has investigated
5 alternatives with adjusting services attending rate and changing the beginning time of the servicer, it appeared that the best alternative which helps reduce the average waiting time is the alternative which changing the starting time of work of
the servicers of card submitting and blood testing step at 06:30 am. Doctors and nurses on the diagnosis and appointment card receiving step also starting earlier
at 08:30 am and change the arrival time of patient at 06:30 am, it can reduce average total time in diabete clinic 112.12 minutes and average total time is
146.15 minutes. The second alternative which helps reduce the average waiting time is the alternative which changing the starting time of work of the servicers of card submitting and blood testing at 07:00 am and change the arrival time of patient
at 07.00 am. It can reduce average total time in diabetes clinic 111.51 minutes and average total time is 146.76 minutes.
