Development of the nursing service for patients on a ventilator in Nakornpathom Hospital Thailand, fiscal year 2004
Abstract:
The nursing service for patients on a ventilator is one of a hospital’s must important
duties. These patients are in a very critical condition and they need close care at all time. In
Nakornpathom hospital this service was one of the main factors in lower respiratory tract
infections. It was found that the nursing service techniques were not correct; there was lack of
good preparation at the start of each shift; and nursing activity processes were not clear or well
documented. This quasi-experimental research, with a two-group pretest and posttest design,
aimed to solve these problems by setting a new working model which was implemented in an
experimental area at Nakornpathom hospital. A similar tertiary hospital was selected to be the
control area. The experiment was conducted for 3 months from December 29, 2003 to March 28,
2004. Comparison between pre and post implementation of the new working model on quantity,
quality, time, satisfaction and expenses was analyzed by Mann Whitney U test, Wilcoxon Signed
Rank test and T-test, at the significant level of alpha = 0.05.
Results revealed that after implementing the new working model the ratio of services
per 1,000 man-minutes of the provider increased (p < 0.001); and the percentage of well prepared
equipment for phlegm suction during night shifts, hygienic care of oral cavities and teeth during
morning shifts and night shifts increased (p = 0.020, 0.025, 0.025 and 0.025 respectively). The
percentage of hands washed correctly before providing nursing services increased (p = 0.007),and
the lower respiratory tract infection rate decreased. The satisfaction of administrators and
providers increased (p = 0.039),It was shown that the new working model had a higher efficiency
and was appropriate for use in the experimental area due to a good and clear working model,clear
guidelines on readiness and preparation before working, a stress on correct nursing techniques,
and good participation by people concerned from the beginning. There was also a manual, which
was simple and easy to use. There were more benefits from utilization of available resources.
Continuity of this model as a role model of organizational Continual Quality Improvement is
recommended. It could be applied in the development of every task in every hospital.
