Use/non-use of hormone replacement therapy in the context of women's menopausal experience : power, knowledge and resistance

Abstract: This study investigated life experiences of Thai menopausal women, their management of menopause, the decision-making about use/non-use of hormone replacement therapy and other alternative approaches and explained their acceptance of and resistance to the medical discourse of menopause. In 2002, I interviewed 22 women in natural menopause from northern Thailand. This study indicated that the menopausal experience reflects the dominance of medical discourse. The medical discourse and discursive practice on the life experience of menopausal women caused the women to perceive menopause as a disorder, and to feel the need to seek medical advice as well as special self-care. Nevertheless, not all of the women were completely dominated by the power of knowledge and behaved as suggested by the discourse. Women still had space to express resistance. This study has found that women’s expression that could be interpreted as a negotiation with or resistance to the medical discourse of menopause are present in various forms including having side effect symptoms, not receiving or discontinuing hormone replacement therapy, (pretending to) forget about hormone replacement therapy and defining menopause as a normal event . I recommend that health policymakers and health service providers should broaden their view of menopause beyond the medical perspective. Medical service should consider women’s need to receive information about both benefits and possible adverse effects of hormone replacement therapy and a woman must be able to make decisions about the use/non-use of hormone replacement therapy.

Abstract: การศึกษานี้เพื่ออธิบายประสบการณ์ชีวิตผู้หญิงไทยวัยหมดประจำเดือน การเผชิญกับภาวะ หมดประจำเดือน การตัดสินใจเกี่ยวกับการใช้/ไม่ใช้ การเลือกชนิดของฮอร์โมนและการเลือกใช้ทาง เลือกอื่นๆรวมทั้งอธิบายการยอมรับและการต่อต้านวาทกรรมการหมดประจำเดือนทางการแพทย์ โดยศึกษาในกลุ่มผู้หญิงที่หมดประจำเดือนตามธรรมชาติจำนวน 22 คน ที่จังหวัดแห่งหนึ่งทางภาค เหนือของไทย ในปี พ.ศ. 2545 การศึกษาพบว่าประสบการณ์การหมดประจำเดือนของผู้หญิงสะท้อนให้เห็นถึงการครอบงำ ของวาทกรรมทางการแพทย์ กล่าวคือผลของวาทกรรมและปฎิบัติการของวาทกรรมทางการแพทย์ ทำให้ผู้หญิงรับรู้ว่าการหมดประจำเดือนเป็นภาวะผิดปกติ จำเป็นต้องเข้ารับบริการทางการแพทย์ และต้องดูแลสุขภาพของตนเองเป็นพิเศษ กระนั้นก็ตามผู้หญิงไม่ได้ถูกครอบงำจากอำนาจของความ รู้และวาทกรรมทางการแพทย์อย่างสิ้นเชิง ผู้หญิงยังมีโอกาสที่จะต่อต้าน จากการศึกษาพบการแสดง ออกของผู้หญิงที่สามารถตีความว่าเป็นการต่อรอง ต่อต้านกับวาทกรรมการหมดประจำเดือนทางการ แพทย์ ได้หลากหลายรูปแบบคือ การเกิดอาการข้างเคียง การไม่รับประทานหรือหยุดรับประทาน ฮอร์โมนทดแทน (แกล้ง) ลืมรับประทานฮอร์โมนทดแทน และการให้ความหมายว่าการหมดประจำ เดือนเป็นภาวะปกติ จากการวิจัยครั้งนี้มีข้อเสนอแนะว่าผู้กำหนดนโยบายและผู้ให้บริการทางการแพทย์ควรจะปรับ มุมมองเกี่ยวกับการหมดประจำเดือนให้กว้างขึ้นกว่าการมองภาวะหมดประจำเดือนด้วยมุมมองทาง การแพทย์เพียงอย่างเดียว และในการจัดบริการทางการแพทย์ควรคำนึงถึงความต้องการของผู้หญิงที่ จะได้รับข้อมูลทั้งในด้านประโยชน์และโทษที่อาจจะเกิดขึ้นได้จากการใช้ฮอร์โมนทดแทนและผู้ หญิงต้องเป็นผู้ตัดสินใจด้วยตนเองว่าจะใช้หรือไม่ใช้ฮอร์โมนทดแทน
