Abstract: ผลการวิจัยพบว่ารูปแบบการนิเทศทางคลีนิกสำหรับหัวหน้าหอผู้ป่วย ประกอบด้วย 3 ขั้นตอนได้แก่ ขั้นตอนที่ 1 การวางแผนการนิเทศทางคลีนิกในระยะยาว ระยะกลาง และระยะสั้น จำแนกเป็น 3 ส่วน ได้แก่ 1) การทำแผนการนิเทศทางคลีนิกโดยรวมทั้งหน่วยงาน ประกอบด้วยแนวทางปฏิบัติจำนวน 8 ข้อ ได้แก่ ต้องมีการทำแผนการนิเทศทางคลีนิกทั้งในระยะยาว ระยะกลาง และระยะสั้น หัวหน้าหอผู้ป่วยเป็นผู้รับผิดชอบให้มีการเขียนแผนการนิเทศในคลีนิก โดยนำข้อมูลจากแหล่งต่าง ๆ มาวิเคราะห์จุดแข็ว จุดอ่อนโอกาสภาวะคุกคามรวมกัน เพื่อการวางแผนการนิเทศทางคลีนิก จัดประเภทของความต้องการการนิเทศทุกระยะ ตั้งเป้าหมายของการนิเทศทางคลีนิก จัดกิจกรรมหรือโครงการให้มีความสอดคล้องกับเป้าหมายและวัตถุประสงค์ของแต่ละแผน และกำหนดช่วงเวลาการจัดเตรียมแผนการนิเทศทางคลีนิก ส่วนที่ การทำแผนการนิเทศเฉพาะบุคคล ประกอบด้วยแนวทางปฏิบัติ 5 ข้อใหญ่ ได้แก่ การจัดทำตารางการนิเทศ กำหนดหัวข้อเรื่อง ตารางเวลาในกิจกรรมการนิเทศประจำวัน และสัปดาห์ นำหัวข้อการนิเทศในตารางการนิเทศมาทำแผน แยกตารางนิเทศบุคลากรทางการพยาบาล การนิเทศการปฏิบัติการพยาบาล และการนิเทศเครื่องมือเครื่องใช้และอุปกรณ์ทางการแพทย์ และจัดทำแบบฟอร์มการบันทึกกิจกรรมการนิเทศต่าง ๆ เพื่อให้ง่ายต่อการเก็บรวบรวมข้อมูล ส่วนที่ 3 การเขียนแผนการประเมินผล การนิเทศทางคลีนิก ประกอบด้วย แนวทางปฏิบัติ 5 ข้อใหญ่ ได้แก่ การกำหนดวัตถุประสงค์และแนวทางการประเมินผล การนิเทศล่วงหน้า จัดให้มีเครื่องมือประเมินผลและรวบรวมข้อ วางแผนการรวบรวมข้อมูลและกำหนดเกณฑ์การประเมินผล การนิเทศที่สอดคล้องกับวัตถุประสงค์ ขั้นตอนที่ 2 การนำแผนการนิเทศทางคลีนิกไปปฏิบ้ติทั้งในระยะยาว ระยะกลาง และระยะสั้น จำแนกเป็น 2 ส่วน ได้แก่ ส่วนที1 การนำแผนการนิเทศคลีนิก โดยรวมทั้งหน่วยงานไปปฏิบัติ ประกอบด้วย แนวทางปฏิบัติจำนวน 7 ข้อใหญ่ ได้แก่ หัวหน้าหอผู้ป่วยจัดประชุมทำความเข้าใขกับบุคลากร ในหน่วยงาน เปิดโอกาสให้ผู้รับการนิเทศมีส่วนร่วมในการแสดงความคิดเห็น จัดผู้รับ-ผิดชอบในแต่ละกิจกรรม และแผนการนิเทศ วิเคราะห์ผลการปฏิบัติการนิเทศ และปรับปรุงวิธีการนิเทศให้ เหมาะสม ส่วนที่ 2 การนำแผนการนิเทศทางคลีนิกเฉพาะบุคคลไปปฏิบัติประกอบด้วย แนวทางปฏิบัติ จำนวน 5 ข้อใหญ่ ได้แก่ หัวหน้าหอผู้ป่วยตรวจสอบเป็นไปได้ของแผนการนิเทศประจำสัปดาห์ และประจำวัน อาจมีการปรับเปลี่ยนแผนหากมีเหตุการณ์ทีเปลี่ยนแปลง มีการบันทึกข้อมูลผลการปฏิบัติวิเคราะห์ผลการปฏิบัติการนิเทศตามวัตถุประสงค์ของแผน และปรับแผนการนิเทศประจำสัปดาห์ และประจำวันเมื่อสิ้นสุดวันและสัปดาห์ ขั้นตอนที่ 3 การประเมินผลการนิเทศทางคลีนิกในระะยะยาวและระยะกลาง ประกอบด้วยแนวทางปฏิบัติ จำนวน 5 ข้อใหญ่ ได้แก่ วิเคราะห์ผลของการปฏิบัติการนิเทศในรูปของ คณะกรรมการ วิเคราะห์ผลของการนิเทศประเมินผลการนิเทศในแต่ละกิจกรรมและ/หรือโครงการ จัดเวลาในการประเมินผล การนิเทศทางคลีนิกเป็นระยะ ๆ และนำผลมาปรับปรุงแผนการนิเทศ เขียนรายงานผลการประเมินประจำปี รายปักษ์ และรายเดือน ส่วนที่ 2 การประเมินผลการนิเทศทางคลีนิกในระยะสั้น ประกอบด้วยแนวทางปฏิบัติจำนวน 3 ข้อใหญ่ ได้แก่ วิเคราะห์ผลการปฏิบัติการนิเทศทางคลีนิกประจำเดือน ประจำสัปดาห์และประจำวัน นำผลการประเมินการนิเทศทางคลีนิกมาปรับปรุงแผนการนิเทศเป็นระยะ ๆ การบันทึกผลการประเมินการนิเทศทางคลีนิกเพื่อนำไปใช้ในการประเมินผลแผนการนิเทศทางคลีนิกในระยะและระยะกลาง

Abstract: The purpose of this research is to develop a model of clinical supervision for head nurses by DelPhi technique, with experts including 19 of chlinical supervision experts and educational supervision experts.The research results found that model of chlinical supervision for head nurses consist of 3 steps 1 the planing of clinical supervision in long-term, medium-term and short-term as follows 1) The planning of clinical supervision consist of 8 procedures such as plan clinical supervision in long-term, medium-term and short-term by head nurses, collect data to analyze weakness and strengthenness for planning, collect data to analyze weaknes and strengtheness for planning, categorize the needs of supervision every period, establish clinical supervision goals, organize activities or projects to comply with goals and objectives of each plan and determine period for preparing clinical supervision. 2) Individual supervision plan consist of 5 procedures such as make supervision schedule, determine titles of schedule in daily and weekly supervision schedule, determine titles of schedule in daily and weekly supervision activities, take the titles of schedule to make plan by devide follow nursing personnel, nursing practice and clinical instruments and make form for recording supervision activities. 3) The planning of clinical supervision evaluation consist of 5 procedures such as determine objectives and guidelines for evaluation ahead set evaluating and collecting instruments, plan for data collecting and determine principles for evaluation and supervision that comply with objectives. Step 2 Taking clinical supervision plan to practice in long-term, medium-term and short-term as follows 1) Puting a whole clinical supervision plan of unit into practice, which consist of 7 procedures such as head nurses organize meeting with personnel in unit, open opportunities for participants to show their opinion, set who responsible in each activity and supervision plan, analyze results of supervision practicing and improve supervision methods for appropriateness. 2) Putting individual clinical supervision plan into practice, which consist of 5 procedures such as head nurses examine possibility of weekly and daily supervision plan, may change plan if situation is changed, record results of practicing, analyze results of supervision follow paln's objectives and improve weekly and daily supervision plan at the last of day and weekend. Step 3 The evaluation of clinical supervisions that are already practiced in long-term, medium-term and short-term and medium-term consist of 5 procedures such as analyze results of supervision practicing by conmittee, evaluate results of supervision in each activities and / or projects, set the time to evaluate clinical supervision and take the results to improve supervision plan, write annual/quarter/monthly reports. 2) The evaluation of clinical supervision in short-term consist of 3 procedures such as analyze results of clinical supervision in monthly, weekly and daily, take results of clinical supervision plan to improve, record results of clinical supervision evaluation to use in evaluation in long-term and medium-term.
