Abstract:
The purposes of this ethnographic future research were to study the structure, factors effecting the structure and elements of the nursing organization, autonomous general hospitals. Thirteen experts from health care reform, health care management and in nursing management, experts selected by purposive sampling. The research instrument was in-depth interview guideline. The obtained data were analyzed by content analysis, categorized, described and verified the organizing of nursing organization, autonomous general hospitals. The research results were as follows : 1. Factors effecting the structure of nursing organization, autonomous general hospitals, consisted of external and internal environment. The external environment were national health reform law, political policy, block grant budget system, consumer needs, technology, hospital size, administration system and service system. The internal environment were human resources and organizational culture. 2. The components of nursing organization should designed by specialization for the quality of nursing services. There should be 2 types of departmentalization, which were functional and according to customers needs. Line of organization must be short, flat, flexible and change regarding to situations. Span of control should be wider and covered both in hospital and in the community. Decentralized decision making should be made by team or committee. Work standard, regulations and effective communication should be initiated. 3.The nursing organization still be existed as one of the department of the autonomous general hospital structure. Head of nursing department must be professional nurse. The structure of nursing organization would be matrix form for effectively mangement in general nursing, specialized nursing and community nursing practice.
